The Pathogenesis and Adequate Treatment of Pancreatic Pseudocyst -According to the clinical study between 1980 and 1988-

Abstract
a The 9-year (1980-1988) experience of 34 patients with pancreatic pseudocysts at Department of Surgery, Seoul National University Hospital has been analysed to investigate the pathogenesis and adequate treatment of pancreatic pseudocysts. The patients were divided into 3 groups as group A, B, and C. In group A, pseudocysts remained stationary or increased slightly with uneventful maturation of the wall, whereas pseudocysts remained stationary without symptoms and decreased in group B. In group C, pseudocysts increased suddenly with suspending rupture or other risk of major complication. The patients in group A were treated by internal drainage or excision and those in group C by external drainage in most cases. Those in group B were observed continuously and spontaneous resolution occurred in all cases. Spontaneous resolution occurred in 6 patients (17.6%, all in group B) and complications during observation period occurred in 7 patients (20.6%).Operations performed in 21 patients and postoperative complications occurred in 7 patients (33.3%). Among them, internal drainage made few complications. The histopathologic findings of the pseudocysts showed that ductal and periductal inflammation was the principal factor in their generation. Though the pancreatic pseudocysts are generally known to be as benign disease, they may frequently cause high mortality and severe morbidity from the fatal complications. Therefore, the importance of adequate treatment in respect to the progress of disease cannot be overexaggerated. Key Words: Pancreatic pseudocysts, Pathogenesis, Adequate treatments
